Our goal at Florida Sports Injury and Orthopedic Institute is to get you back in the game. In order to do this we need to work WITH YOU to ensure that you are involved in the management and improvement of your health.

We would like for you to access your data / medical records / new patient paperwork though our HIPPA secure website. In order to verify your identification we need you to click on the portal link below and enter your first and last name, last 4 of your SS # and DOB. Once verified, continue to log in and complete the registration.

For existing patients this is a quick and easy way for you to get all your medical records / radiology reports / orders / and or direct access to ask your doctor a question.

For all new patients you can use the portal to complete all new patient paperwork, this way your physician can review your history and completed medical record prior to your initial appointment.
